FREQENTLY ASKED QUESTIONS
What happens at the first appointment?
After an appointment has been requested, intake paperwork will be sent for the patient to fill out before the appointment via a link to a secure patient portal. At the first appointment, we will go over information to verify accuracy, review questionnaires and paperwork, and go over the confidentiality statement.
